Hospice Services

It is the hospice philosophy that the physical, emotional and spiritual needs of terminally ill patients and their caregivers can best be met by the services of an interdisciplinary team of trained professionals and volunteers. VNA Home Health and Hospice nurses provide skilled nursing care including physical assessment, patient and family teaching pain and symptom control monitoring and a registered nurse on call 24 hours a day.

Individuals needing help with daily living tasks will benefit from personal care services provided by Home Health Aides. These certified aides assist with bathing, dressing and food preparation under the supervision of a Registered Nurse. Psychosocial needs arise from an individual's illness and affect both patient and family. The hospice Social Worker can provide counseling and access to appropriate financial and community resources. Bereavement Services are an important part of hospice care. The Bereavement/Spiritual Care Coordinator can arrange clergy visits at the patient's request and provide spiritual counseling and bereavement services to patients and families. Our Hospice program offers ongoing bereavement support groups for adults and children. Extensively trained Hospice Volunteers are available to individuals and families for emotional and practical support, as well as respite care and friendship.
