We provide high-quality, cost effective alternatives to institutional care, by delivering essential healthcare services all in the comfort of your home. Our multidisciplinary team, led by a nurse or therapist, and with the direction of a physician, works together with the patient and family to promote independence. We help you and your loved ones feel more comfortable, safe, and confident by providing loving, comprehensive care in a familiar setting. Our care will be scheduled to meet the demands of your healthcare challenge, and coordinated with your family members, when possible. A registered nurse is on call 24 hours a day. We are just a phone call away!

Cardiac Care
Wound Care coordinated by a Certified Wound and Ostomy Nurse

Falls Risk Assessment and Prevention

The mission is to help patients achieve the highest level of independence possible by helping them recover at home. The focus is to provide individualized rehabilitation services specifically tailored to each home environment – either as an extension of, or an alternative to, institutional care. Together with the patient, his or her family and physician, our team will develop a rehab program designed to meet each individual's needs and goals.
The multidisciplinary home care team includes:
Case Managers
Registered nurses who provide skilled care and coordinate care
Certified Nursing Assistants/Home Health Aides
Under the supervision of a registered nurse, assist with activities of daily living such as bathing, dressing and food preparation
Dietitians
Provide nutritional counseling
Physical and Occupational Therapists
Assist you back to your optimal level of functioning
Speech/Language Pathologists
Assist you with regaining or improving your speech
Medical Social Workers
Help identify and manage psychosocial problems that arise from an individual's illness or medical treatment; provide counseling and assist with access to appropriate financial and community resources.

Who pays for home care?
Medicare, Medicaid, and most private insurance plans pay for services that home health agencies deliver. Payment from these sources depends on whether the care is medically necessary and the individual meets specific coverage criteria. Individuals may opt to pay out of pocket for services that are not covered by other sources. When services are medically necessary, and there is no ability to pay for the services, the VNA provides Financial Assistance Programs.
